Please find registration materials below. The deadline for registration is January 1.
We will accept forms after the deadline, but space will fill-up. The forms are accessible and can be completed using a Smartphone. Once completed, you will receive an email confirmation. This is not an official calendar, but only a confirmation that your request has been received. Your “official” seasonal calendar will be mailed / emailed after January 4.
FIRST, complete the Participant Registration Form for Winter-Spring 2017
THEN, complete the appropriate form based on your primary residence:
East Side of Cleveland including eastern Cuyahoga (including Lake, Geauga)
West Side of Cleveland including western Cuyahoga & Lorain County (including Medina County)
YELP and PARALYMPIC TEAM SPORTS Registration Form for those who wish to participate in organized Paralympic Team Sports or YELP Programming for participants age 16 and older.
Forms WILL NOT be processed until BOTH the Participant Information Form and Program Registration Form have been completed. Please pay close attention to age restrictions and transportation options when signing up.
- ABOUT YELP: The goals of the Youth Empowerment & Leadership Project are to empower participants to become more self-sufficient and independent, to help participants with physical disabilities and teen volunteers develop leadership skills, to encourage community service, and to help youth of all abilities tap into their own unique capabilities. In order to help those who take part in programming meet these goals, YC offers a number of leadership, service, sports, and educational programs. Through YELP, YC participants, volunteers and alumni are focusing more on the necessary life and leadership skills that will help them succeed.
- ABOUT PARA TEAM SPORTS: YC Paralympic Team Sports provide participants the opportunity to develop their physical skills, compete recreationally and competitively, and actively engage in a team atmosphere. Through weekly practices, participants develop as athletes while building camaraderie and friendships among teammates and volunteer sport assistants. YC Para Team Sports help to teach our athletes the importance of hard work, dedication, and overcoming physical challenges in sport.

For those who are more comfortable with a written format, you may request a paper form which corresponds with your home location: East, West & Lorain County, or YELP & Paralympic Team Sports registration.
